Application Form Questions
Overview
Application Form Questions are the individual questions that make up an Application Form. They are candidate-facing and are organized into sections within the Application Form. Questions can be configured to contribute to a candidate's Lead Score, helping end-users identify which candidates are the best fit for a position.
Every Application Form must contain at least one section with at least one Application Form Question.

What are Application Form Questions?
Application Form Questions are the questions candidates answer when submitting an application for a position. They are organized into sections within the Application Form and are used by submissions to generate Lead Scores for candidate submissions.

Who Creates Application Form Questions?
Application Form Questions are created by users responsible for configuring Application Forms for a position. You must have the appropriate permissions to create, edit, or delete Application Form Questions.

Where are Application Form Questions Created?
Application Form Questions are created within sections inside the Application Form tab of the Position details page.

When are Application Form Questions Created?
Application Form Questions can be created or removed at any time. At a minimum, the Application Form must always contain at least one section with at least one question before a Position can be published.

Why are Application Form Questions Important?
Application Form Questions configured with weighted answers generate Lead Scores for candidate submissions. Lead Scores give end-users an objective signal for determining which candidates are the best fit for a position — without having to manually review every submission individually.

What Types of Questions are Available?
Application Form Questions can be any of the following types:

| Range | Candidate selects from a predefined set of ranges (e.g. "1 year of experience" as minimum, "10 years of experience" as preferred) |
| Single-Choice Selection | Candidate selects one answer from a list of options |
| Multiple-Choice Selection | Candidate selects one or more answers from a list of options |
| Free Text Reply | Candidate provides an open-ended written response |
| Photo or File Upload | Candidate uploads a file (supported types: .docx, .doc, .pdf, .jpg, .jpeg, .png, .webp) |
| Video Upload | Candidate uploads a video (.mp4 and standard selfie-video formats from desktop or mobile are supported) |
| Date Selection | Candidate selects a date from a calendar input |
How Do I Create Application Form Questions?
There are several ways to add questions to an Application Form.

What is the Question Library?
The Question Library contains a set of pre-built questions organized into four categories. Questions from the library can be added directly to any section of an Application Form without additional configuration.
Note: The ability to create your own custom question libraries is coming in a future release.
The four categories are:
Information Capture — questions designed to collect general candidate information such as resume uploads, video introductions, start date availability, and self-provided transportation.
Candidate Preferences — questions designed to understand candidate preferences such as travel availability, remote vs. onsite preference, work schedule availability, and salary expectations.
Skills & Experience — questions designed to assess candidate qualifications such as years of relevant experience, technology proficiency, language proficiency, and licensure or certification uploads.
Candidate Verification — questions designed to verify candidate eligibility such as drug test consent, citizenship eligibility, sponsorship requirements, clean driving record attestation, and background check consent.

What Question Configuration Options are Available?
Each custom question can be configured with the following options:
Screening Question
| State | Behavior |
|---|---|
| Enabled | The candidate must answer the question. Their response must match a preferred answer for the submission to be marked as a Match. If it does not match, the submission is still collected but marked as Not a Match. |
| Disabled | The question is not required. |

Tear-Off Question
| State | Behavior |
|---|---|
| Enabled | The candidate's response is hidden from all non-admin users. Used for compliance-sensitive questions such as DE&I reporting, veteran status, or disability status. |
| Disabled | The candidate's response is visible to all users. |

Weightless Question
| State | Behavior |
|---|---|
| Enabled | The question is excluded from the Lead Score calculation. The candidate is neither rewarded nor penalized regardless of their response. |
| Disabled | The question is included in the Lead Score calculation. |

Demographic Question
Note: This option is only visible if your organization has an Indeed Job Post API integration enabled.

|---|---|
| Enabled | The question is tagged as a demographic question for Indeed's compliance and record-keeping requirements. This is a backend tag only — the candidate experience is not affected. |
| Disabled | The question is not tagged as a demographic question for Indeed. |

Can I Rearrange Questions or Sections?
Yes. Each section header and each question title has a grip handle. Click and drag the grip handle to reorder sections or questions within the Application Form.
Can I Edit Existing Questions?
Yes. Existing questions can be edited at any time by users with the appropriate permissions.

Can I Delete Existing Questions or Sections?
Yes. Questions and sections can be deleted at any time by users with the appropriate permissions. At a minimum, the Application Form must always retain at least one section with at least one question.

Can Application Form Questions be reused across multiple positions?
The Question Library is standard and available across all Positions. For questions that are not in the Question Library, it is recommended to import an existing Application Form from another Position rather than rebuilding questions from scratch. This ensures consistency across similar roles and saves time when creating new Positions.

What Happens to Existing Submissions if a Question is Deleted?
Deleting a question triggers a new draft version of the Application Form. If that draft is published, the updated Application Form becomes the active version and candidates will answer the newly configured set of questions going forward. Existing submissions collected under a previous version are not affected — they remain tied to the version of the Application Form that was active when they were submitted.

Can Sections be Renamed?
Yes. Sections can be renamed at any time without affecting the questions contained within them or any existing submissions. It will trigger a new draft version of the Application Form however.

Is there a limit to how many questions or sections an Application Form can have?
No. There is no hard limit on the number of questions or sections an Application Form can contain. However, it is best practice to keep Application Forms concise — ideally between 1 and 15 questions. Shorter forms generally result in higher completion rates and more complete submissions.
